JA-SIG conference registration fee includes all conference program materials, admission to all conference sessions, refreshment breaks, reception Sunday evening, light breakfasts each morning, luncheons on Monday and Tuesday, and a Monday evening dinner and reception.

Please fill out one registration form for each attendee. You will receive an email confirmation letter at the end of the process.

Cancellation
Policies
Payment for the full amount of registration fee must be made prior to or on the first day of the conference.

Payment may be made by credit card (Visa, MasterCard, American Express, or Diners Club). If you do not want to submit your credit card number over the Internet but still want to pay for the conference by credit card, complete the registration form online and then call us with your credit card number. Please note that your credit card statement will reflect that your JA-SIG conference registration fee has been charged by CONCENTRA-CMS, JA-SIG's Conference Management Company.

If you wish to pay with a purchase order, please select the "pay by check" option and then type the PO number where indicated on the form. If you prefer to pay by check, please mail a check payable to "CONCENTRA-CMS" to:

To qualify for the early registration fee, we must receive your registration form and payment no later than November 6, 2003. All registrations received after that date will be processed at the $325 registration fee.

Cancellation Policy: All refunds are subject to a cancellation fee. Prior to November 6, a cancellation fee of 10% of the processed registration fee will be deducted from refunds and after November 6, a cancellation fee of 50% of the processed registration fee will be deducted from the refund. There will be no conference refunds after the start of the conference.
